When a task is marked as Completed, the dynamic form configured to appear on task completion is not recorded in the Activity History. The tasks have been completed directly without any form interaction, despite the process being configured to display the form on task completion.
RCA #
The task was completed using Bulk Actions from the Manage Tasks page.
Bulk completion does not trigger the task-level process or form display, so no submission activity gets recorded.
Corrective Action: #

Disable bulk task completion within the process configuration to ensure the form is always shown during task completion.

Restrict user permissions via Permission Template to prevent users from completing tasks via Bulk Actions, forcing completion through the standard flow where the form is displayed.
