There is an issue where duplicate opportunities are getting created even if they are satisfying the duplicate detection rule(DeDup Rule).
This is caused due to the setting in the Forms named as “Allow Non-Lead Owners to Create an Opportunity” wherein the non-lead owners can create an opportunity for inaccessible leads via Forms where the dedup rule is not triggered. Since, the deDup rule is not triggered, therefore duplicate opportunities are getting created.
Please note that this setting will only show up in the form once “Allow Users without lead access to create opportunities” setting is enabled in Advanced Configuration.

2. If the dedup rule was modified recently

3. If both the opportunities ; existing and duplicate are meeting the criteria set in the deDup Rule.
4. If there’s any extra space or special character in the field values which are included in dedup rule.
