Form not getting Deployed on Add Opportunity

Issue:

This Form is deployed on Add Opportunity and it’s visible on the Support access. However the same is not visible either on the user account or Admin Account.

– The Form was not displayed on both Old and New UI.
– In process Add at a Specific work area even  is added.

Solution:
#

Check if the below configuration is added :

Go to My Profile>Settings>Leads>Lead Details View.
For the above case, Opportunity Button is not enabled through Lead Details View in the Settings. 
After adding this Button, the form reflect on UI.
