Here the Customer mentioned that a particular lead unsubscribed but post that the lead was receiving the one-on-one and from campaigns.
When checked we got to know that the category from the email campaign in the first step as attached in the below screenshot newsletter or promotional, if they unsubscribed either of these then the lead will still receive the other 2 categories type (None, newsletter/promotional)
Note: Always suggest client to keep blank while running a campaign. This way if a lead unsubscribes a single mail, it will get unsubscribed from all the campaigns which will sent in future.
